Recent advice is that all pregnant members of the public should self-isolate. Your Nanny should check this advice is still current using https://111.nhs.uk/covid-19.
If the medical advice is for the employee to self-isolate then this is paid as per your contractual sick pay policy which is normally SSP. Proposed recent changes to legislation mean that SSP is likely to be payable from the first day of sickness rather than fourth.
